For and in consideration of the acceptance of relief offered to me by the Carnival Corporation pursuant to the Settlement Agreement between the United States of America and Carnival Corporation dated ___ day of ______, 2015: I, _____________________, release and discharge the Carnival Corporation, its subsidiaries, affiliates, insurers, successors and assigns, and its current, past and future officers, directors, shareholders, employees, and agents, from all legal and equitable claims related to my complaint, DJ No. 202-17M-206 concerning the Carnival Corporation’s alleged failure to provide individuals with disabilities equal access to its ships and facilities, programs, and activities in violation of the Americans with Disabilities Act.
This Release constitutes the entire agreement between Carnival Corporation and me. This Release will be considered null and void in the event the Carnival Corporation fails to send me and I fail to receive a check in the amount of $______within thirty (30) days of Carnival Corporation’s receipt of this signed Release.
I acknowledge that a copy of the Agreement between the United States and the Carnival Corporation has been made available to me. I further acknowledge that I have had the opportunity to review the terms of this Release with an attorney of my choosing, and, to the extent that I have not obtained that legal advice, I voluntarily and knowingly waive my right to do so.
I HAVE READ THIS RELEASE AND UNDERSTAND THE CONTENTS AND DECLARE UNDER PENALTY OF PERJURY THAT THE FOREGOING IS TRUE AND CORRECT.
