The chart below lists the facilities surveyed and gives an alphanumeric code for the required remedial action. The key to these codes is found in Attachment M to this agreement.

2. Route from Parking to nearest Entrance: The route is inaccessible because there is a 1 inch change of level where the asphalt pavement and concrete sidewalk meet. AR1
3. Entrances:  
a. The building does not have directional signage posted at inaccessible entrances and does not have the International Symbol of Accessibility posted at accessible entrances. D16
b. The door is inaccessible because there is a slope of more than 2% within the door’s required maneuvering clearance. D13

1. Parking: The designated accessible parking is inaccessible because the sign is mounted too low, lacks a “van-accessible” sign, is not located on the shortest accessible route to the designated accessible entrance and has a surface that is not firm, stable or slip-resistant. P3

8. Gladewater Former Students Building, located at 2509 Hendricks, Gladewater

a. The parking lot, with a total of 163 parking spaces, is inaccessible because it lacks a van-accessible space. This lot requires 1 van accessible space and 5 standard accessible spaces. P3
b. The designated accessible parking, numbered 1 through 6, from left to right, as you face the building, is inaccessible because the access aisles of spaces 2 and 3 are too narrow. P8
